KMC provides individuals with a process to request access to personal information held about them and request correction of inaccurate or incomplete information.
A request may be made by:
the individual concerned;
an authorised representative;
another person where legally authorised to act on behalf of the individual.
Requests should be sent to:
KMC Privacy Officer
Email: connect@kiwimedconnectglobal.com
The request should include enough information to help KMC identify the person and locate the relevant information.
KMC will assess the request and respond in accordance with the Privacy Act 2020.
Where appropriate, KMC may provide:
a copy of the relevant personal information;
an explanation of how the information is used;
information about the source of the information where required or appropriate.
KMC may take reasonable steps to confirm the identity of the requester before providing personal information.
KMC will not request unnecessary identity documents.
Individuals may ask KMC to correct information that is inaccurate, incomplete, misleading or out of date.
The request should identify:
the information believed to be incorrect;
the proposed correction;
supporting information where reasonably necessary.
If KMC does not agree to make a requested correction, KMC will explain the reason where required and, where appropriate, provide a mechanism for the individual's statement of correction to be recorded with the information.
Where information relates to another individual, KMC may need to consider that person's privacy rights before releasing information.
KMC will not charge a fee unless permitted by applicable law.
KMC will handle requests within the timeframes required by the Privacy Act 2020.
If an individual is dissatisfied with KMC's response, they may contact:
